- Participants: who is available to be tested and the city or ZIP code for each person.
- Purpose: whether the report is for a court, agency, or another official use.
- Timing: any court order, deadline, or other case requirement.
Please do not leave identification numbers, copies of court papers, or DNA reports in an initial message. We will explain what documents are needed for your case.
